For federal taxes, you'd need a form 1040, a schedule C or C-EZ to show your income and associated expenses, and a schedule SE to calculate self-employment tax. The numbers from the bottom of the schedules will transfer to your 1040.

We need more information. Many states and local communities have their own laws about this, and may require training, background checks and licensing. Our state exempts from this if not more than (I think) 4 children are cared for in the home, but allows local government to impose stricter requirements.
