God's chosen people were/are the Israelites who followed Judaism... they are the direct descendants of Jacob/Israel. They remain God's chosen people....

God sent Jesus and all who follow Jesus are called Christian...

but it is one God, creator of heaven and earth

Well, many different peoples became christians. First Jesus came to the Israelites though. But he also went to any others that would hear his message.
