I would say Regan was the best because his policies took out several small tyrants. There were a lot of worst. Truman's policy help start the Cold War, JFK almost caused a nuclear war, Jimmy Carter nearly invented Arab's bloodlust against the U.S. and then there was that whole make friends with anybody and let the Africans starve policies that many presidents had.

Modern day? How far back are you talking?
The last good foreign policy president was FDR. Knew how to keep an alliance together. Knew how to run the military - by putting good men in charge and letting them run things their way. Was a master of deception, keeping the full extent of his illnesses hidden. Kept that maniac Stalin at a respectful distance. Even Reagan had grotesque failures ("Irangate") and so did Clinton ("Rwanda"). Nothing THAT terrible on FDR's record.
The worst? In all seriousness, the current one will be talked of as such alongside LBJ ("Khe Sanh") and Nixon ("Cambodia"). Close tie between the three is what historians will likely declare.

Clinton's was the worst; and I say that because his policy was buy everything with our tax dollar. His administration was about the photo op in the white house with people of countries all over the world. They had no respect for the US; and in many cases the money was used inappropriately too.
The best was Ronald Reagans. His mere acceptance of office ended the Iranian Hostage Crisis; he brought an end to the threat of a soviet \ American war that would have quite possibly been the end of man, and there's just no way anyone is going to compete with that in terms of foreign policy.
