Depends on the agency - in general, you fill out a form asking all kinds of questions - have you ever taken anything w/o permission, have you ever had a bad job review, have you ever used drugs, when was the last time you used drugs, have you ever gotten a speeding ticket, etc.. etc..,. maybe 10-15 pages of questions.
The examiner then goes over the questions/answers with you to clarify & understand your answer. You are then hooked up to the poly and asked most (if not all) of those questions. The combination of verbal responses and the reactions on the poly will generally give a good indication as to whether you're being deceptive or not - no, it's not foolproof.

I've been given a polygraph test when interviewing to work at a security company. First (without the polygraph) they ask you all kinds of questions about any past crimes, drug use, your application. Then during the polygraph, they ask you one by one whether your answers to the previous questions were true.

They will usually ask you about 20-25 questions in the pre-test phase. From these questions the tester will choose about 7 or 8 along with a couple of general questions such as: Is your name ____?, Do you live ____?. Those questions are there to set a norm. The test is usually administered 3 times.

I had a polygraph test eons ago for a job. I was told I was TOO honest. WTH was that suppose to mean? Fortunately though I was able to rectify that fault & have lived happily ever after...
