The head of my penis has been really red in some spots for the past 5 months. I got some anti fungal cream to put on it. It helped take it away only to come back in a week or so. Every now and then it really gets red and I get like a red rash in my foreskin area. What is this???

A diagnosis can be made only after seeing the rash. Most rashes have a very specific appearance so they need to be seen to be diagnosed. Please go see a doctor.
