I have taken her to the vet and got an antibotic for a bladder infection but she still has the same problem. She can be sound asleep and wake up with a big puddle under her. After I wake her up when she does the involuntary urination she still has to go outside.

If the visit to the vet didn't solve the problem, take her BACK to the vet! If they don't hear from you, they're going to assume all is well! You need to at least phone them and tell them what's going on. A different approach is needed.

If it is incontinence (likely in an old dog, their sphincters become weak) there are meds that can help manage this.

Certain urinary problems can actually be quite difficult to diagnose.

In the meantime:

- Monitor exactly when the dog is urinating
- How much volume/consistency/time is being lapsed in between.
- How much water intake she's receiving daily.

Also have the veterinarian test the urine for other possible problems such as kidney/bladder stones and ask him how long the medication should take to work.

If it's not working let him know! Also never be afraid to ask him any questions or concerns that you have about your dog's current condition. As an owner, you have every right to know exactly what's going on.

If this problem is consistent, I would also get a second opinion from someone else - possibly a specialist.

You didn't give her age. Maybe the medication needs to work a bit longer or a different medication. Good Luck. I know this upsets yo but she cannot help it, don't be mad at her. it is a possibility that her kidney's may be failing but this is highly unusal for younger dogs. I hope the vet gets a urine sample and tests it.

perhaps it's another problem have you gotten the dog spayed lately, I've heard a small % of dogs can have urination problems after being spayed, check out this website, if it is confirmed to be an infection then medicine takes time to work.

The incidence of estrogen-responsive urinary incontinence is increased among neutered female dogs, and there was concern that neutering puppies at an earlier age might further increase the risk for spayed bitches. Urinary incontinence in dogs neutered at traditional ages can develop within days of the surgery or not until several years later. Estrogen-responsive urinary incontinence was reported in 34 of 791 (4%) bitches neutered at conventional ages, and seven of 2,434 (0.3%) sexually intact bitches[126127]. Howe et al., [130131] evaluated 269 dogs adopted from shelters and neutered at < 24 weeks of age or > 24 weeks of age and found only three cases where owners reported urinary incontinence. One dog was neutered at < 24 weeks of age and two dogs were neutered at > 24 weeks of age. Thus, there is little information to date to suggest that the incidence of urinary incontinence is higher in bitches spayed at earlier ages over conventional ages.

Could it be incontinence?? The vet can give you something for incontinence.. If she has no infection, and still leaking, I would investigate other avenues with the vet..
