Have you two had a good talk about where you are, financially, socially, mentally? How old are the other children? and how old are the two of you? How many other children do you have? Are you healthy, secure, faithfull? So many questions. But only the two of you can decide if its right for the both of you.
As to how to ask, just like anything else - go to her at a relaxed peaceful time and say "honey, I've been wondering how you feel about ......" and take it from there. Be prepared to answer any questions she might have for you.

If you already know that she does, then ask her about a time line on when. If she doesn't care and says now, go online to an ovulation calendar and see when she is the most fertile. Then make a romantic dinner and set the scene. Send the baby over to grandma's for the night and then get on with it.
