You don't have to do anything. It's your 401k contribution, and has already been taken out of your box 1 income that you'll report on the 1040EZ line 1

Good news and bad news. Depending on your AGI, you may be able to get a little of that back. SInce it is listed as "D" you will not be able to do a 1040 EZ, but you will have to complete a 1040 A. It is not much harder, especially if you are doing it electronically, but in order to get the credit for your contribution, you will have to fill out a 8880. You will more than likely get about 10 percent give or take back from it.

A D in box 12 indicates the amount of your 401(k) contribution.
