If you have the one for your records still, you can send that to state, if not you will need to get a replacement, I think if you look on the net, you can find an 800 number for the state tax board, or look in your phone book, under state, in California, state of California, then go to franchise tax board, then find 800 number for personal income tax, Cal # is 800 852-5711, other states will be listed where ever you live. tell them what happened and they will tell you what to do at this late date.

You can ask for another copy, but it's probably too late to get the copy. In the meantime, go ahead and fill out the state taxes and send in to avoid penalties. Presumably you have all the required data as you have a copy of your federal return.

if you have the last check stub for the year you can use that to fill out your taxes. how did you do your taxes, if online there could be a copy, like on turbo tax online. if you have your federal copy just make a copy. as long as it is the same as the federal , it really wont make a difference what is says at the bottom. like state referece copy, federal copy, etc.

Asking the company for another copy would be a real good place to start.

Yes, ask the employer for a duplicate copy.
