Hi,
have completed the 1040EZ, but I do not know which other form I need to complete. I am preparing this form my brother. Please let me know.

The tax form 1040EZ is complete in itself. There is no other form to submit with it, unless you are talking about attaching a copy of his W2 to the side of it before you mail it in. Other than that, just keep a copy of the return for your brother, and mail the original one in.

Do the instructions say you need an additional form?
The only tax credits you are allowed if you use 1040EZ are the EIC credit (worksheet is in the 1040EZ instructions) and the telephone tax credit which you don't need a form to take'
If your brother can or wants to itemize deductions, take any other tax credit or file as head of household he must file form 1040 and not 1040EZ.
You can always call the IRS and ask them if you don't understand something. if your're not trying to do something shady don't be afraid to call them at the number in the booklet.
You can also get info at the IRS website www.irts.gov.
If you are doing something shady, calling them isn't a problem but they will catch up with you eventually.

Are you talking about the state tax form? Your state may have income tax forms to fill out as well. You will need to check with your state department of revenue as to which form is correct in this situation.

You probably don't need any other form. If he has some kind of income or credit that doesn't go anywhere on the 1040-EZ, then he'd have to file a 1040A or 1040 instead.
