We addressed our outer envelopes formally, but did our inner envelopes informally, listing the names of all the guests we were inviting.
For instance, the outside would be Doctor and Mrs. John Smith, but the inside would just be John and Jane. If they had children we were inviting, it would be John and Jane on the first line, then John, Julia and James on the second line.
For family members, we did things like Uncle John and Aunt Jane, or Grandma Joan, and people seemed to like it. My MIL commented that it was a nice personal touch to do that, and several of their friends had told her that they liked getting a more personal invitation.

How close are you to these people?
If you're social, but not very friendly, then both envelopes should be Dr & Mrs John Doe.
If they're family or very close friends, then (what I did anyway) you should go with Dr & Mrs John Doe on the outer envelope and John & Jane Doe on the inner envelope.

Him no longer understanding their better halves in my opinion has no longer some thing to do with this. in any respect. The better halves want to be protected on both the outer and inner envelopes (and in case you have not already ordered them yet, i ought to easily pass the interior envelopes because that they're an entire waste of money). The invitation should be addressed to Dr. and Mrs. John Smith. in the adventure that they are both medical doctors, it would flow to The medical doctors Smith.

Keep it as Dr. and Mrs. John Smith, unless they have children and the letter applies to the whole family, in which case The Smith Family would be more appropriate

Dr. and Mrs. ____ is the formal way. However, if they are really close to you informal might be nice. I’ve always called my dad’s best friend ‘Uncle’ and in my wedding invitations I addressed the inside to Uncle _______.
