I think you are asking what a theraputic INR is when you are taking a drug called coumadin. INR is a number that is created from the time it takes your blood to clot. People walking around with typical blood have an INR about 1. When a doctor gives you coumadin it is usally to get your INR to about 2 or 2.5 so that you will not have clots forming and going to parts of your body that it may dammage.

Stents are placed in the circulation of the heart to "open up" the vessel to let your heart get more blood.

Bypass is when the surgen goes in and takes vessels out of your legs and places them in your heart to have your heart get more circulation to the parts that need it.

A defibrilator is to treat the electrical activity of your heart if you have a life threatening rhythem of ventricular tachycardia or ventricular fibralation the defibrulator will shock your heart and possibly save your life if you need that. It is implanted under your skin and is a relitively less invasive surgery than the bypass.

Because of all the heart problems you stated, the medication called Cumadian is routinely given to heart patients especially one with a "stent",it keeps the blood from getting too thick or clotting to easily to prevent clots to the lungs and brain, that would be fatal in most cases.I don't know if you mean a defibrillator that's implanted in a patient or the ones in a heart attack emergency kit? The same reason would also be indicated with the implant, prevention of clots, and thinning the blood. All patients taking blood thinning meds must have there clotting time repeated often, the INR or PTT & PT what regulates the Coumadin dosage Take care.
