just ask the doctor. say that you require a note saying it is chicken pox and he will fill out one of their official notes with her name and chickenpox as the cause of illness. he will of course also sign and date it.

for the first 7 days you can sign your child of sick without a doctors note anything after that requires a doctors note but im sure the doctor will be happy to write one for you
