just ask the doctor. say that you require a note saying it is chicken pox and he will fill out one of their official notes with her name and chickenpox as the cause of illness. he will of course also sign and date it.

Ask him for a medical certificate, stating that the child has the chickenpox.

for the first 7 days you can sign your child of sick without a doctors note anything after that requires a doctors note but im sure the doctor will be happy to write one for you

just say ur child requries a sick note , expline that ur child needs one because of the lenth of time he/she has been of school

why would u need one? surley u word should be enuff for anyone if they dont beleive u tell em to sod off.
