Usually I can translate pretty efficiently, but the syntax of the question is confusing me somewhat.

That question is so easy. It's "What can I help you with?" You sure you can translate pretty efficiently? If you did, you should have understood it off the bat. No offense but maybe you need help with your english too. Try looking up efficiently in the dictionary.

What would you like me to do? What can I do for you?

Sometimes the syntax does seem scrambled, but the secret here is in the verb poder, used here as puedo which means it is you who performs the action and te is the objective, so you are doing it for someone else.

How may I help you?

"en que. . ."= how or in what way
"atender"=assist
The comma seems misplaced.
