You know the couple. What makes them unique? What do they love to do?
My husband and I love to whitewater raft and go camping so we would have loved (even though we didnt register at a camping store) if someone would have taken the initiative to get us something for camping.
Another couple was planning on adding a deck to their house and received a gift card from home depot along wtih one of their books on simple deck plans.
It really depends on them and what is really something the 2 of them like to do. Are they outdoorsy? Do they boat a lot in the summer? Would they really love tickets to a play that is coming out in a few months? An off-broadway thing? A year pass to a museum or zoo? new jerseys for a local sports team if the old jersey is wearing out? Is one of them obsessed with their car? As you are a bridesmaid, I would think it best to get her something that you know about her, something that perhaps only you would think to get them, rather than just pick something off the registry. However, if you can't come up with something creative on your own--perhaps make a basket with all the cheap items that are often overlooked on registries. We had a lot of lil $2 - $5 items that no one bought. Get an easter basket and wrap some of those lil items and the couples favorite candy. Or if the 2 of you are really close friends, do somethign a lil risque that no one else would dare to do. Perhaps a sex game, some hersey's syrup, a giftcard with a list of lingerie shops to try (check out www.trashy.com), some massage oil, a copy of kama sutra. http://www.kamasutra.com/
One of my bridesmaids make a scrapbook of the wedding planning sessions, the dress try-ons, the bridesmaids try-ons, the bachelorette party, the nail salon and the pictures from the wedding and reception that we'd posted online. It was great and definitely something completely from her!
Either way only you truly know your friends. Come up with something from you. Something that only you would know to get her. That will be what she will cherish and unlike just another set of china, she will remember that it came from you!

Here's something creative that will be appreciated and you will have fun doing:
Make up a crossword puzzle and have all the hints be personal inside moments and jokes that only you and they would know about. Also things to do with their relationship and wedding planning. Put it on a poster board and add some clipart and some pictures to it!
The crossword puzzle could be time consuming and a bit frustrating ... you could do the same idea with a WORDSEARCH or WORDFIND puzzle.
Put your computer skills to work! Use Excel, get clipart from Yahoo Images Search, copy them, and reformat them, use a gluestick to put on posterboard.
Use graph paper, and pictures from magazines if you are not as good on the computer, and it will give it a homemade feeling.

Are they going on a honeymoon? Do you know where they are going to be staying? Then call the resort and arrange for a special dinner or maybe pay for the "romantic at sunset sail"...usually someone at the hotel can make some suggestions.

like another writter said if you know where they are going why not book something there if not make a basket and fill it with things like a kinky basket an put things in or a night in with bath robes slippers tea bags movies cups a couple game from like spencer gifts.
a gift card for flowers or plants for the home

Hmm... Maybe but them a nice bed sheets. Or a wedding stuffed toy like.. 'I'm Married'. Or give them both a voucher for the best resturant or hotel or a vacation or something for them two. (But be careful, that will be alot of cash!)

You could make a scrap book of their lives growing up and then of their life together. They will be able to have it for years and years and share it with their kids, etc.
