Did you want a "low-fat" smoothie or a "low-calorie" smoothie?
In general, what makes something "fattening" is the number of calories it has, but "healthy" foods are also low in fat and sugar (as well as sodium -- and high in vitamins, minerals, fiber, etc.).
So, a very tasty "low-fat" smoothie could be blended up from plain yogurt (I make mine with 2% milk) plus frozen fruit, but you'd still need to add some form of sweetener (sugar, honey, maple syrup, etc.) probably, and the sweetener will add more calories (the smoothie would still be low-fat, but somewhat "fattening" depending on the proportions in the drink and total amount drunk).
(Some people don't mind artificial sweeteners, so if you used artificial ones the total calories of the smoothie would be cut down --but they taste terrible to me).

If you like a frozen coffee, this is good--my own recipe. Use cubes of frozen leftover coffee as your base. Pour in enough evaporated milk to suit your taste. Sweeten with your choice of sweetener---I like Hershey Syrup--a good no cal sweetener is stevia powder. Add whatever you want--vanilla, cinnamon, whatever you like & blend to a slush consistancy. SOOOO yummy, refreshing in hot weather, hardly any calories and cheap. What more could you ask for?

just use some orange juice, fat free yogurt and some frozen or fresh fruit and put in the blender and you have a really awesome drink with very little fat.
