Procrastination is a hard one to fight, but as for concentration, you need to pace yourself. Study for no more then 70 minutes without a ten minute break, as the brain winks out with too much work. Also, be well-rested and well-fed. Lastly, you need to be in the same state as when you will take the test or write the essay or whatever. State Dependent Theory says that you remember more information when you are in the same state of being as when you learned it. If you are tired when studying, it is best to be tired when taking the test. If you were high while studying, then it is best to be high before the test. It has to do with how your brain encoded information. Best of luck.

Write on a 3 x 5 card "Do you want fries with that?" and every time you feel your concentration slipping, read it aloud.
