Even among those Churches founded by Christ and the apostles in apostolic succession the form of the bread varies. It is not so important that the form be the same but instead it is important that one discern the confected bread and wine as the Body and Blood of the Lord as taught by St. Paul to the Corinthian Church. According to Scripture, to do otherwise is to bring condemnation and possibly death upon oneself.
In the first century Church only the bishop could confect the Eucharist and only those Baptized could even be present during and after the consecration. Even among the baptized they were told to examine themselves as to whether they were worthy vessels for the Lord with no unrepentant sin as they were to be in a state of grace.
It is in the Mass that the whole Church, Militant, Suffering and Triumphant celebrate the one sacrifice for all by our Lord and Savior being obedient when He said, "do this in "anamnesis" of me".
As Scriptures say not to partake in the real Body and Blood of our Lord is to be without life eternal and separates one from the covenant that Christ made possible with mankind.

Jesus turned the water into wine, thats how I consume the liquid part. And I whip up a batch of home made bread once in a while.

Well, for 1 thing, if any1 kept in line with the precedent Jesus himself set they would use unelavened bread (referred to by the Jews of today as 'matza' [sp?], the typical cracker-like bread eaten during Passover).
2nd, only the few who are under the special covenant with Jesus that are to rule with him as kings and priests in heaven are to partake of the bread and wine.
