Create a newspaper front page and include funny pictures and stories from her life. I can send you a basic copy if you are interested. then frame it and have it on display.
Also buy a nice note book and pass it around at the party for her friends and family to write comments in. This was done for my 30th and I love it for all the sweet sentiment at the begining and all the drunken ramblings added at the end.

You could have a fancy dress party but all the characters have to be from the late 60s and make it a reunion type thing where you get some of her old school mates etc.

A friend of mine had a fancy dress party for his 40th recently and he had old video clips of him when he was a child running in the backround and music from that era with photos of him growing decorating the venue. It was really good!

What about photo collages. Large Construction paper in nice bright colors, have people over about a week before party, such as friends and cousins or whoever might have some cool photos of her throughout the years. Separate the photos into years or stages throughout the years and have everyone make there own collage after the photo's are separated then use them as decorations. We did this and it was such a hit.

Have the people put there names on the photo's if they want them back afterwards.

I'd make posters....and have fun with it....say for example....one on red poster could be theme Boy was HER face red, no idea if she is married or not but if so...a white poster for wedding pics...decorated in her wedding colors(ex bells in wedding colors, a sweet 16 poster, a pink poster for baby pics, school pics, old boyfriends, first apartment, graduation, her kids

and I'd have fun with the meal preparation.......for example.....a menu of 40's........40 cabbage rolls, 40 egg rolls, 40 meatballs(just an idea but you get the picture)

Or something else that I think might be fun....since you mentioned having pics and things...you could do a sort of "this is your life "theme.......and have everything 60's...maybe show a movie from the 60's, have 60's music....dances from the 60's , people dress from the 60's era....etc etc

I had a surprise 50th and my sister made the favors. She
made a CD for each guest. It was all of our favorite songs, and the cd had printing on it with my name and "50th Birthday Party" and the date.
Also, there was a poster sized photo of me (when younger)on an easel where you entered the room.

For my sisters 30th we hired a hall and did a party. i had a photo printed onto her birthday cake of her aged 4!!!
We also got loads of past photos scanned and blown up into posters and put them up on the walls. we also had streamers, balloons, banners with happy 30th on (you can get them for 40th)
You can even get her a flashing crown and wand om ebay with happy 40th on
Your party invites could have a photo of her when she was a little girl on it! So personal.

Get 40 ballons, attach one pic to each and have them floating from the ceiling or rising from floor.

Have family and friends fill out 40 index cards(not each-collectively as a group) with things they like/love about your sister, place them in a basket or index card box that has been decorated.

Take her to an exotic island pretending not to remember anything about her birthday. And there a lot of friends and acquaintances will meet her all of a sudden.

yes just let her think you have forgot her birthday all together that way she wont be suspicious as to if you are organising something to suprise her.
