Loss of lung compliance.

Without surfactant, the surface tension increases and impairs elastic recoil... and would in turn prevent normal lung expansion. This would result in increased work of breathing.

A lot of neonates have problems with lack of surfactant. They often use artificial surfactant to improve lung compliance.
