It has nothing to do with good hygiene. It's caused by an organism that enters the body; it isn't washed away with soap and water
In some stages of the disease, condoms will prevent transmission very effectively, if used correctly. If it breaks or comes off, then there is a risk.
If a person is symptomatic, with wart-like lesions on the genitals, it can be spread through the skin. Unless the condom covers all the lesions, it won't be effective.
Avoiding contact with an infected person is obviously the best way to go. Consistent use of condoms is a very good idea. Most people with syphilis will not have any symptoms.
If a person knows that they have been exposed, syphilis can be prevented by a course of penicillin.
