The principal action of nitroglycerin is vasodilation — that is, widening of the blood vessels. The difference between the drug and the explosive is the concentration of nitroglycerin used and the nonexplosive starch filler or fixing agent that keeps it stable. The nitro spray agent or the nitro pills that people put under their tongues during an attack of chest pain are not going to explode. And no, nitro pills don't cause any sort of "micro-explosion" to open the arteries of the heart. The main effects of nitroglycerin in episodes of angina pectoris are:
subsiding of chest pain
decrease of blood pressure
increase of heart rate

Nitroglycerin relaxes the veins that take blood to the heart. This takes some of the stress off of the heart and helps nutrients (oxygen and others) get to the heart.

Mechanism of action (straight from the notes we were given on Nitrates in my cardiovascular module class last week):
"Selective direct-acting smooth muscle relaxant"
meaning that it causes vasodilation, reducing resistance to blood flow, thus causing angina to subside. Vasodilation also causes BP to decrease.
