does anyone know what would cause kittens to be born with their intestines prolapsed (hanging out)?

Nobody knows the cause of why there are abnormalities in fetal development like this. In humans sometimes you can blame medications like Thalidomide or viruses like measles or bacteria as in syphilis for abnormalities in fetal development. For cats, probably viral infections especially respiratory which is the most common problem can be a cause. But I do not know the history of the mother of this kitten. Sometimes without any obvious reason, an abnormal development of the fetus like septal defects of the heart, or no anal opening just comes out. Probably it can be a problem of both the genetic source (egg or sperm?) which might have been of poor (weak) quality so that during development something goes wrong . This intestines hanging out at birth are one example - this one the abdominal wall fail to completely close allowing the internal organs to escape outside.

They could have been in extreme distress in the womb/during birth, causing a rectal and eventually intestinal prolapse. Or, it could be a congenital abnormality.

Either way, if you have a kitten in this condition, please get it to a vet NOW, as it won't last long like this, it will get an infection very quickly if it hasn't already died of shock.

It's a perineal hernia. I would recommend you don't breed with this cat again, have her spayed as it is thought to be genetic and you find future litters will have the same problem.

If you haven't already, take it to the vet and it can either be treated or euthanased depending on the severity.

It's unfortuantely just one of those things it happens with human babies as well it's not common but not much can be done with kittens.
