I wanted to know who sends off the letter, and who do we send it to? Do we get permission first from the insurance company , is that who we are sending the letter to? If you want the insurance to pay for it, then the Doctor needs to deem it necessary to continue to live healthy and/or call your insurance company and see if your coverage allows it, then you just go to the appointment. Before you go in please research this, there is a diet you need to be on before you go in, it helps prepare you intestines. Also if the Doctor just ushers you in and wants to get started, STOP, this is a very serious surgery and most "good" doctors will send you to a seminar before your intake apt. so that you know ALL the risks, then there is the diet afterwards, you eat something that is not allowed and you could die from internal bleeding due to staples opening... there are three different types to think about, the band method is less evasive and just as effective. Your intake on food will be limited: 2 tablespoons is all your stomach will be able to hold afterward, which is why you follow the "before-surgery-diet". There are guidelines that the Doctor performing the surgery will go by to see how "in-need" of this procedure you are which is the information the insurance company will need for the coverage question.
