I'm the same way! (Only, I'm not a student anymore!)
The ONLY thing that works for me- setting an alarm!
If you have a cell phone, set reminders for when you need to do things. If not, get a timer that you can set for 60 minutes or less- then do whatever it is you're supposed to do!
Enlist friends to help you- they can even call you at certain times (like, 7pm, so and so calls to tell you to do your geometry homework, or whatever).
Enlist your parents- it is kind of their job, right?
Make a 'NO Rule': No TV until _____ is done. No Video Games until you've done ______. No internet until you ______ this.
And don't worry- it takes doing something at least 21 times to make it a habit.
