My son works part time in a grocery store. I filled out a 1040ez form but it only asks for federal information. What form would we use to file his state tax? Sounds silly to ask this but I normally don't do taxes.

File Ohio IT1040EZ.
Fill in the blank PDF: http://tax.ohio.gov/documents/forms/ohio_individual/individual/2006/PIT_1040EZ_SFI.pdf
Instructions: http://tax.ohio.gov/documents/forms/ohio_individual/individual/2006/PIT_IT1040%20Instructions.pdf

You can also go to:

and I-File for free but, if he is a first time taxpayer he will have to register with the state.

Note - Ohio will let him claim an exemption for himself even if you claim him as a dependent on your taxes.

To answer that question, would need to know what state you live in.

You can check your state's website - it probably has information on taxes, including forms to use, and might let you efile on the website.
