Yes. I'd go back to the dentist who did the RCT. Sometimes a tooth is so infected that all of it can't be removed. Of course, this isn't intentional. Another possible reason, is that there are more root canals that expected. Maxillary (top) teeth have 3, mandibular (bottom) have 2. Sometimes there are more, although it's uncommon. Go back and they can take an x-ray and tell you the exact problem, I'd bet it's b/c of one of those reasons.

yes it is possible. If your tooth had multiple canals then the dentist could have missed cleaning a canal to remove infected root. The dentist can re due the root canal without removing your crown.

yes-root canals are breeding grounds for bacteria-i have never had one for that reason-always had my tooth extracted and placed an implant-very safe-
