I dont know if you can find them, but im sure that if you find a custom cabinetry or staircase shop and im sure that they can either forward you to someone who can or can make them for ya

Try an Architectural Salvage Company if you want something Authentic ,they go to Demolition Sites to Acquire Old Fashioned Materials, Stone work, Flooring , Bannisters,Lamps etc,It depends on how much you want to pay. There is Plenty of Companies who make New Bannisters. Try Yellow Pages.

Yes, depending on whether you want to match or make all knew. You would be better off to find someone who does wood working on the side. Lowe's/Depot also carry a small variety to choose from.
