Well highlighting is bleaching, right... it's damaging. I'd say do it as rarely as you can get away with it. When roots show badly then it's time, but overall bleaching hair damages it.

Once every oh, 2 or 1 and a half months. Too often might make your hair fall out. Also, only touch up your hair when the color is fading.

it depends on the color, your natural hair color (how well that matches with the highlights), and how fast your hair grows.

you should highlight your hair when the roots come in so it does not look trashy (not syaing you are or any thing it just does)
