I just reinstalled Windows XP Home with an Admin user and several Limited users. I spent a few hours installing several apps under the Admin user before using any of the limited users.

When I log in to the limited user, a dialog box pops up. It's only message is "fAIL". A item appears in the windows taskbar with a icon I don't recognize.

Can anyone tell me any of the following:
1) What app does the icon belong to?
2) Is there any way to identify what process/program is creating that dialog box to troubleshoot/remove it?

Turns out I solved it. An application related to my CF card reader was doing something you couldn't do as a limited user. I removed the device - never use it anyway.

I narrowed it down by looking at all the startup programs and shutting them off one at a time til I found the offender.

Boot into safe mode and scan your computer for virus activity and malware/spyware activity. Try also going to Bitdefender.com and doing an online virus scan. This looks suspiciously like a virus message to me, especially considering the way the text is formatted.
