Most moonlighting is done by doctors who have done enough training to be licensed practicing physicians but are still in a residency or fellowship for additional training. Training programs don't pay well at all (salaries divided by hours invested are often less than minimum wage would be if paid on an hourly basis, pretty sad for somebody with that much education). There are also those who just want to pick up some extra cash, and since doctors have a reference point of 80 or more hours per week from their training, when they get into private practice, the 50 or so hours they work seems like nothing, so an extra 24 hours' work in a week seems like a snap. It's also not uncommon for some to work extra to support an ex-wife or two, which is pretty common, not to mention a vicious circle, considering the hours.

"Moonlighting" is when you work after passing the Board Exams (after getting your pfofessional license): you don't go to specialization right away, instead you work as a General Physician. You can work in clinics, companies, etc.

Doctors who moonlight, are usually residents that are working as physicians in other things besides their residency.
