has anyone known an airbag to activate unexpectedly while driving?

This has been heard of, but typically it takes the equivilant force of driving into a brick wall at 18 mph, or into a parked car at 24-28 mph.

Not while driving but I have seen one get set off by an electrical disturbance caused by a remote starter. It takes a pretty significant force to set them off, I have seen them set off by people hitting curbs as well but never heard of one just randomly going off unless it was sabotaged by someone.
