To my knowledge, the only one, and it is not a true opiate/opioid, is Tramadol. But it IS a semi-synthetic, atypical opioid. There are some states that are considering making it CV, however. To address the poster below me, nutmeg is NOT an opiate/opioid in any sense! In extremely high doses, it MAY produce effects similar to ecstasy, which is not derived drom the opium poppy - neither is nutmeg. This is not "Yahoo Guesses"- we should strive fot accuracy!

The best responcbe only got 1 thumbs up by me. Kratom is the only organic/natural occouring opioid we know of besides e the opium poppy. It has psychoactive effects as a stimulant at lower doses and binds to the mu and delta opioid receptor; bigger the dose = more binding to the mu-opioid receptor. Immodium is an semi-synthetic opioid that does not cross the BBB, ever taken too many opiates and been constipated?? Well they jsame idea only since no crossing of the blood brain barrier - you get no psychoactive effects, thats the whole point and reason it's OTC, if you take ungodly ammonts you will catch a buzz but is it worth being more constipated than you've ever been in your life (Minus having to drink liquified charcoal - if you've been there you know what I mean) and it will even potenitate the effects of most opioids (As will proglumide, cyclizine (aka marzine but out of production, was a great antiemitic or vertigo meicine; off-label use is as an opioid/opiate potentiator.
The drug Diconal is a combination of cyclizine with the opioid dipipanone.
Dipipanone is a schedule I controlled substance in the US, due to its high abuse potential due to its abuse with methadone & the combination medicine Bonine for kids now out of production and boxes of 8 go from 20 to 120 on amazon used, just to show how well it works) , hydroxyzine (aka atarax/vistari), promethazine all potentiate opiates, helping them metabolize faster and moreso than usual (Thus why Tussinoex is hydrocodone and chlorpheniramine, or codeine and promethazine syrup)

Opiates? Tramadol is an opioid, not an opiate, the difference being an opiate is something in opium. And opiod is something that mimics morphine, or more specifically, something that binds to opioid receptors. In the US, no opiates that have an opioid action are not controlled.
There are a lot of opioids that aren't controlled drugs, however most of them are not clinically or recreationally useful. For example, the endogenous opioids beta-endorphine or dynorphin are not controlled. Likewise synthetic opioid peptides like DAMGO are not controlled. As said before Loperamide is not controlled, but does not pass into the brain at any reasonable concentration in most people.

There are only a few. As stated above, Tramadol is a non-scheduled opioid however it is categorized as prescription-only. Also, Loperamide is a non-scheduled opioid and is even available over-the-counter however it does not readily cross the blood-brain barrier so it does not produce any analgesia. In addition, the herb Kratom (Mitragyna speciosa) contains alkaloids that readily cross the blood-brain barrier and bind to opioid receptors. Kratom is not controlled in any way by the government and is available at several on-line vendors. And Nutmeg is definitely not an opiate.

Dextromethorphan is somewhat related to opiates but is not a controlled substance. It does not have the typical actions of opiates, except for cough suppression.
