most commercial courses teach us the grammer only

Though there are many school/colleges for English speaking courses. But you can get some idea from there. Actually you will have to practice to speak english then and only then you can be fluent in english. First of all you should not hesitate. You can speak in english with a younger one. Now a days kids can speak english if they are studing in convent school. You can speak in english with them. That will make confidence in you and you will not feel shyness to speak with them though you are not speaking correctly. But you will feel your mistake and you can remove that by consulting book or any senior or teacher. That way may make you fluent in english.

Let me give you my experience with Sign Language instead...

I worked at the National Technical Institute for the Deaf...

When it is a matter of "if you don't learn you can't communicate" one picks up a language VERY fast!

At NTID we started by taking an INTENSE course:
8 hours a day,
5 days a week,
for 3 months!

I do not know which country you live in, but you might want to see if they have a:
Berlitz School of Languages in your area.

I used Berlitz for Arabic.

All I can say you is that the best way of speaking fluent english and that of better standard is to read books.It can be any books....any novels or still better News papers.
They use dialogues and thus help you in framing your sentences when you speak.Reading gud books also help you to add humour to your speech thus making it more appreciable and attractive.But reading is not all!You also gotta inplement it.Once you start reading , make sure that you also start talking.Instead of simply sitting in the vehicle reaching ur school/college imagine a conversation and start trying to frame sentences and proceed the talk.It'll help you a lot.

Spoken English is very different from the written one , as we know!
The best way to acquire spoken English skill is to speak it...get the inhibition out of you.
Listen to BBC if you want the RP or if you prefer American English...try movies.

take it as a habit and ur fluent at spoken language.
let the English language go into ur subconscious and u will automatically speak English. good luck

Practice makes Perfect =P

I did a lot of practice: story-telling competition and singing at school, reading story books and whole lotto stuffs...
