There is no real need for sexual alternatives.
People with STD's or AIDS can and often do have a normal sex life.

These kinds of choices come down to the people themselves.
There is no hard and fast rule that regular sex must stop.

Ideally, two people in a relationship make a decision about what their comfort levels are.

For instance, someone may have an STD and their partner doesn't. They may decide that all sex that they have will be done with a condom.

Some people may decide that everything they've heard about an STD isn't really as bad as what they've heard once they know the facts and realize it's a stigma, and decide to carry on normally.

And some people may have the same STD as each other and decide that there is no need for an any alternatives since they've already got what each other has got, therefore, they don't have to do anything differently.

Outside of the normal things that these people do, then any other alternative sex that a person can have is limited only to your own imagination.

Da best alternative is to not have sex at all. Your best bet is to masterbate to a picture of them
