Medically no it's not. If your healthcare provider asks you if you are sexually active, they're asking about actual sexual intercourse and not masturbation. There aren't really any risks involved in masturbation, while people who are sexually active have a risk of getting STD's, HIV, etc.

No because the phrase sexually active means "having sexual intercourse". To be sexually active you need a partner.
