It's important that you and your grandfather get educated about this disease. Ask the Dr. when you see him tomorrow if there is anyone you could see or anywhere you can go for this purpose. If not then get a book. The more you know about this disease, the healthier your grandfather will be.
Since he only ate toast and a banana, would I be right in assuming he may be sick? Illness, stress or trauma to the body could make blood sugar levels rise regardless of what was eaten.
Only your Dr. can tell you what to do about it.

It sounds like he definitely has diabetes and should be on insulin. Anything over 100 is worrisome. At 375 he might need to go to the emergency room. Call the ER in your hospital and ask them if 375 is serious or could it wait until the next day to see his doctor. Does his insurance have an advice nurse phone number you can call?

toast does have carbs which are formed into sugar, but if that is all that he has eaten today that could be why the blood sugar is so high. Insulin is secreted to break down food and to keep energy up, If your grandfather is not eating healthy meals at least three times a day that could cause a rise in his sugar.

Yes, he is going to need insulin. That 375 is super high. Many people have to go to the hospital at that level. It is not easy to explain why it is so high. It is part of the type 2 diabetes cycle.
There are various devices which can make it easier for older people to give themselves insulin injections.

Get your grandfather to a doctor.
How much plain toast, how large a banana did he eat half of? When did he eat them?
I'm not a doctor, I can't diagnose or prescribe. But, I can recommend that you get him to a doctor.
