Depends on the STD. AIDS and syphilis are detected by blood tests.

Chalmydia and gonorrhea are detected by testing directly cervical mucus (ie a pap test) or penile secretions. Herpes and genital warts are diagnosed primarily by visual inspection of the lesions.

Herpes can be detected by blood or swabbing lesion.
Chlamydia/gonorrhea can be detected by urine if it has been at least 2 hours since you last peed or by swabbing inside of penis/vagina. Men usually don't like the swab!!
Syphilis (spelling?) blood test for some reason is called a RPR.
HIV/AIDS blood test.

You can tell if you have syphillis, hepatitis A,B, and C, and HIV from a blood sample...other diseases such as chlamydia, gonnorhea, among others, are identified by secretions from the genitals.
