First you would call the paramedics immediately! You always want to try and catch her before this happens - if you see her getting dizzy or extremely agitated (these are signs that her sugar is dropping) - give her a glass of juice or a piece of candy to get her sugar up. If she is in a coma already though, not much you can do until the paramedics arrive.

If your grandmother does "pass out" it is generally due to low blood sugar.
Learn the signs of hypoglycemia (low blood sugars).They include: Fatigue, feeling unwell, nervousness, irritabilility, confusion, sweating, paleness, dizziness and hunger.
If your grandmother is showing these signs, you need to check her blood sugars. If she is low, give her some sugar (juice or pop are good sources) and some carbohydrates (bread, crackers).
Hypopglycemia, if untreated, can lead to unconsciousness. If this happens, catch her, lay her on her left side in the recovery position, and call the ambulance.
Some people with diabetes carry around an injection called glucagon for this type of emergency. If so, you ccan ask your grandmother to show you how to use it.

You need to start reading up on the subject and know what sort of insulin your grandma uses and how much,and if she's on a sliding scale of regular insulin and know the dosages by heart. For a diabetic coma,you never give her sugar,her blood sugar is way too high,that's the reason she's in the coma.Just call 911 and they will do the rest,you could check her blood sugar,to see how high it is she will always have a "acetone "or fruity breath odor(,the big difference between coma and shock)so they will have something to go by.The opposite of diabetic coma is insulin shock,that's when you would give her some sugar under the tongue never down her throat,she's liable to choke.Still call 911, a women her age should always be evaluated at the ER
