as i can only speak personally. in no particular order:
obesetiy
anorexia or a woman who looks painfully skinny and boney.
smoking is a HUGE turn off as is if she were in to heavy drinking (an alcoholic (more than one or two alcoholic drinks per day).
ilicit drug use is also a turn off. (prescription and over the counter medications are of course excluded from this when taken for the purpose they are designed for) recreational use of medications is a HUGE turn off to me.
more than two or three small tattoos and or more than a couple of piercings again are a turn off.
as is bad breath but then i've never encountered any woman with bad breath.
poor personal hygiene is also a turn off as is a woman who over grooms and uses more make up than she needs.
a woman who can not go more than a couple of hours without makeup and or other cosmetics is also a turn off.
turn ons a well groomed woman who is happy with who she is without the need to use cosmetics or the other chemicals that the companies con most people in to buying (you know what i mean, the bull **** the firms use to sell products like anti-ageing creams and other crap like the chemicals to hydrate the skin etc).
a woman should be that, a grown woman has hair on the leggs and other places normally and naturally. there is nothing wrong with a woman being 100% natural. by that i mean No make up, no waxing herself anywhere, no tweezing her eye browse etc.
all you anyone really needs to do is eat a healthy well balanced diet, drink plenty of fresh clean unflavoured water (none to very little alcohol), wash and bathe at least once a day and proper oral hygiene. do that and not only will you be happier in the long term but your bank bal;ance will be better off.
other turn offs include if she is a fashion victim and does not have her own sense of style.
if she gets what is allegedly the norm or the celeb thing to do etc from magazines or the tv rather than her own personal opinon and sense of style etc is not good either.
may be a woman does not exist who does not believe in cosmetics and does not change her wardrobe and sense of style based on what is alleged to be the latest fashion according to the fashion editors and fashion designers etc.
and may be that is yet another reason why i'm still single at age 33.

Biggest turn off is easy, constant calls when you are fishing, at the match,or busy. Just because you aint on the blower every ten minutes dont mean you aint thinkin of them or at it with someone else!!!

Flakiness, insencerity, really bad breath, being too demanding, riddled with anxieties and insecurities, too preoccupied with their looks, infidelity, drug using, zero confidence, um, that's all I can think of right now! If it makes you feel better most girls avoid these pitfalls!
