My advice is to take the thorn out with a sewing needle if you can cope with it, otherwise you can try tweezers if you have them.

WAX sorry not nair

Get the kind meant for removing hair from the
skin. You'll find it in the toiletries section of the drug store, near the Nair
and razor blades. It melts at a comfortable temperature, and doesn't get
brittle too fast. The microwaveable kind is the easiest to work with. Test the
temperature before you start - it should be warm, but not hot, and it should be
about the consistency of honey. If you can tell which direction the stickers
went in, put the wax on against the grain of the stickers, let it cool a bit,
then pull it off WITH the grain. It should get most of thorns out, provided
there's a piece protruding above the skin's surface. It'll also take most, if
not all of the hair off, too. Don't worry about this, though. You can pretend
you did it deliberately because you're a Serious Cyclist. I'd strongly
recommend that you enlist a volunteer to help you with this project. If you're
at all furry, you may not be in the mood to proceed after the first strip or
two.

Well, that depends on the cactus. Some thorns have barbs on them (like fish hooks). I would advise you to have a doctor take it out. At least that way, you can be sure that pieces of the thorn won't be left under your skin. A doctor has the magnifying lenses and teeny forceps to remove it with. A doctor can also numb the area with a topical anesthetic (like benzocaine) to keep the removal as pain free as possible.
