This is an awkward situation on it's own without adding the fact that the guy is her best friend's brother. In my experience he may only see her as a little sister because she is his sister's best friend.

Men smile at women for one of two reasons. He smiles at you because he thinks there is the potential and believes you are a special person or he is just very nice and he smiles at you because you are his sisters friend and is probably nice to most everyone. Personally, I believe that he could be into you, especially if you are as much alike as your friend says. Next time, just kinda smile back and engage in small talk to kinda feel him out and then continue on a gradual basis and eventually the realization should come clear. I hope this helps.

Its hard to answer without knowing how old your friend is. Three years is a lifetime if she is 14 and he is 17, but less if she is 18 and he is 21. I wouldnt tell the sister that she likes her brother, if he only likes her in a brotherly way it could make everyone uncomfortable. Guys can be shy... but I think if they are really interested in you, they will find a way to let you know.
