Is it not then truth at all? Does believing something to be true justify stating said belief as Truth or Fact? Should a phrase like " I believe " or " I have faith that " always be included when speaking or writing about said personal truth?

Truth is truth. It is not relative. It is impossible to say this is truth to me and the exact opposite is truth to you. It doesn't matter if it's "to you" if it is not true. Both can be wrong, but both cannot be right. If someone says "2+2=5 to me", they may be sincere, but they are wrong. 2+2=4 and all the sincerity in the world will not change that fact. Just because you may believe something doesn't make it true. Truth is absolute.

well, this forum is about personal truth. and who ever told u this "my truth is not the same as your truth" they are right. because it's what they believe to be truth. and maybe you will disagree with their truth. that's why it called PERSONAL truth when a person states " I " believe or " I " have faith that... good question though

Yes. For example, someone who is not homosexual may say that they believe that homosexuality is or is not inborn. For someone with a homosexual orientation it is a fact that homosexuality is inborn. One thinks s/he knows the truth, the other does know the truth.
