Have you made it clear the current medication or dose isn't working? If so have they explained their reasoning for not making a change?
You have to be sure to speak up and make yourself heard. Don't be mean or angry but be clear and to the point. They see many, many people so you have to take it upon yourself to make them understand your condition. .
You also have to be prepared for their explanation, should they give it to you. You might not like what they have to say but if they give you a reason for not wanting to change your medication or dose you have to accept they know what they are doing and have good reason for their decision.

Yeah, why just try to help the pain, you need to get to the cause. anti-inflammatory's are the drug of choice for INFLAMMATION. MRI sounds like a good place to start. Sometimes they'll inject joints too. Don't try to drug yourself up, it's not the answer for chronic pain. Maybe something to sleep at night, or an antianxiety medicine.

Get an MRI to see if you have a rotator cuff tear, don't rely on narcotics.. you may need surgery.

Explain just what you said to him. If it doesn't help, go to new doc
