I really can't stand people staring at me while I'm eating, and I hate it when people get food around their Mouthe's, it makes me feel sick.

Rotten service, awful food that wasn't hot!
My worst meal ever was a spinach and blue cheese pizza in Boca Ratton, Florida. It was made in a wood fired bread oven and should have been delicious but it was watery and the spinach was tough....such a shame because the service was great and the wine delicious.
I just came back to edit this...I also hate waitresses in the States that need to know your name, get in your face and keep bugging you to take more drinks when you are having fun talking to family...it's so false!
I don't know anywhere else in the world that needs tax on top of the meal and then 15% tip. In Spain you pay according to the service you recieve and that is usually discreet service!! :-)

Popular or new, hot spots making it difficult to make reservations.
My all time worst experience was at a restaurant in Napa (Bistro Jeanty) about 7 years ago. It was fairly new and just gotten on the SF Chronicle's Top 100 List. We wanted to celebrate our 10th anniversary w/ friends so my husband called 6 months ahead of time to see about making reservations. He was told that the restaurant didn't take reservations that far in advance and that he should call back in 3 months. He called again & was told to call 30 days before the desired date. He did.
We went to Napa to spend the weekend. In the meantime, the restaurant called our home to confirm. Of course, we weren't there & we did not expect a call.
My husband called the day of the dinner to confirm and check. The person said everything was fine.
We showed up w/ our guests & they had given away our table! They said they called our home & when my husband told them he called that morning, they asked who he spoke to.
They didn't bother to apologize or send over a little something, etc.
This is the all time worst restaurant experience. We have never experienced anything like it before or since.
On a brighter note, the next evening we went to the CIA @ St. Helena without reservations on a whim. Although we had to wait, they were very friendly and even sent us a free dessert for our anniversary.

I have many problems with eating out--I am in the business, and my husband and I have gotten to the point where we'd rather stay home, I cook and we get more wine.
I don't like servers who forget they are working in the "service" industry--I understand that this job probably isn't going to be your career, but don't be a b$%^#
I also hate it when you go to what is supposed to be a decent restaurant and the food is not made on premises--LAZY--I'd rather wait and have something delicious.
I also hate how casual everything is, I don't think it would hurt anyone to dress up.

Probably a tie between a crying baby, and a really loud table nearby.
People, you can't bring an infant into a restaurant, amongst loud noises, strangers, weird smells, and they are strapped in one spot where they can't move around for a long time, and not expect them to cry. Then, you don't do anything about it until you get a critical mass of dirty looks from everybody around that is having their nice dinner out that they are paying for ruined by the screeching wail of an infant. For crying out loud (ha ha) get a sitter. If you can afford dinner out, you can afford a sitter.
Next Item: REALLY LOUD CROWDS. Sometimes I am fairly certain that these people are loud simply to draw attention to themselves. Guess what guilty parties: you aren't impressive with your big social circle and unfunny jokes. We aren't fooled into thinking you are sitting it out on an injury list simply because you scream the loudest at the football game on the tv. You are simply obnoxious and ruining my dinner. Nothing about eating dinner requires laughing so loudly that I can hear it from the parking lot. Do you shout down your friends when you are at work? How about your mom? Do you talk at her so loudly that people 10 feet away can't hear each other when talking normally?
Restaurant Yellers: Please shut your pie-hole.
That's what I hate most about dining out!! Thanks for stimulating a nice chance to get that off my chest.

People talking loudly on their cell phones, especially when it is innapropriate conversations.
Children whom were never taught that restaraunts (along with planes and movie theaters) are not the place to get out of their seat and run amok, along with the parents who induldge such "antics."
Overcooked food, because you know sending it back will put everyone behind a good 15 to 20 minutes and it just isn't worth waiting on a new one.
Splurging on something, say a good steak, and finding it to be poorly cooked, seasoned, and/or all gristle.
Loud, obnoxious music (like crappy new age jazz that sounds best suited for elevator muzak) piped over the speakers.
Want me to go on, lol?

Several things. I hate rude and forgetful waiters. I also hate when something is cold or undercooked or wrong. I went to a restaurant last weekend with my gram and our salads and soups were brought out after our dinner!
I also found a piece of plastic in my greenbeans. It was alright as far as mistakes go (and we got free dessert!)

A waiter/waitress who gives lousy service (especially screwing up my order), and still thinks you "owe" them a tip. I've gotten to the point that I just call in my order and take it home; that way, I can check it before I take it, and get what I want, and not have to deal with a snotty server who thinks they're doing me a favor by showing up for work.

Being seated near screaming children. I once had a kid fling food in my hair at a restaurant, and his parents didn't blink an eye or apologize.
