Don't see him alone
Don't go out with him 'one more time' for closure.
Don't phone him or take his calls or texts.
If he is truly persistant or threatening tell him to stop or you will take out a restraining order. Then do it.

Booby-traps. If you're out and you see him spying on you from a table, start talking to a guy you meet and after a time, tell him this guy keeps staring and following you that you went out with once and that it's creeping you out. tell the bouncer that he followed you to the bathroom and asked for your number and that he's trying to grab you all the time after telling him to get lost. Have a cop stop him and ask him for some ID...there are lots of ways!
