Sorry but the answer is YES. That is if you're arriving directly from Hong Kong or Macau. Even if you travel via a Chnese city (for example Guangzhou) you wil need to go through entry formalities at Haikou or Sanya Airport. If however you travel DIRECTLY from another city in China there are no Border entry formalities.

No - Hainan island is off the south coast of China and it is surrounded by the South China Sea only. The nearest country (after China) is Vietnam which is to the West. I do not think there are any direct connections with Vietnam yet.
