In 2006, I worked in UT, FL and MN. But my employer whose office is in MI gave me W2 for MI and MN state. So, I need to get the tax back from MI and also pay to UT state as it is not paid.
In case, if need to file for an extension then with which states I should file?
How to get tax refund from MI?
Appreciate, if get an answer to this.

File a non-resident MI return, listing $0 income and the MI taxes withheld. You'll get a full refund of them.

File non-resident returns in the states where you worked but did not live, listing the income you earned in that state and pay any tax due.

File a resident return in your home state lising all income earned in all states. You will get a credit against your tax liability for the taxes paid to the other states.

You need to file a non-resident form in the state you worked but did not live in. your state tax return usually asks the question did you pay taxes to another state. so do the non resident forms first. Sometimes they are available on line.

ask your employer why the w 2 is missing from UT. sounds like maybe you need to head to H&R Block (or some other tax prep place) since your return in complicated by so many factors. btw, filing an extension doesn't mean an extension on paying what you owe, just on filing the return. get help and get it figured out asap- just over 1 month til tax day. good luck!
