I think this is a great opportunity to try out your (or a close friend's) creative side. You can buy a standard guestbook, and get some brown/gold leaves from a craft store, along with some other autumn themed decorations, and put together the perfect guestbook. As for the pen, you could make your own. Buy a long desk pen and add some artifical flowers that match your flowers you are using to decorate or carry. Instead of a guest book, you could use a 11 x 14 photomat and also decorate the corner with some flowers and have all your guest sign it...then afterwards you add your favorite wedding photo and put it on the wall to see forever instead of put away so know one will see except when you pull it out. You can try to look at Davids.bridal.com they also sale accessories or you can just google wedding guest books and you'll get several hits. However, my suggestion especially if you're on a budget is to go to Hobby Lobby. They have a decent selection. What you can do is get a plain one then decorate it with faux leaves and all kinds of fall things. If you can't do it, find a crafty friend. Because really, the guest book stays open most of the time and hardly no one ever sees the thing so don't stress about it.

Most card stores, or florists have them. Otherwise try something creative. I didn't have a guest book, We took polaroid photos of everyone, had them sign the bottom and put them into a photo album right there on the spot. This way we not only got to remember who was there, but see them too. It was a ton of fun. You can do this as people enter the church, exit the church, or enter the reception. IT is a blast. Everyone got to have their picture with their family or guest and it added a little bit of extra memories to the day.

Well honestly we bought a plain one for our wedding from a craft store and decorated the front with fake flowers that matched the colors in our theme. It was hand made, inexpensive and still very elegant maybe you can look into doing something like this for yours.

I had quite a hard time with that myself. We ended up finding one by Beverly Clark. It's simple white with silver leaves.
I searched around a little, and couldn't find the exact book but it was very similiar to this:

go to a stationery store or paper store. Look for blank autograph books. Get a nice pen and there you go.... They have many blank books that might be able to fit your theme

I got mine at a christian book and gift store.....also you can think about making one....buy a plain white guest book and dress it up with a nice ribon.....
