Normal air sacs (alveoli) have a springy action/reaction. Cigarette smoking destroys the alveoli. It makes them larger, floppy, and less elastic. Not only will they not move air very well but they also lose surface area with this disease. There is no cure for emphysema. Only managing the disease with medications, oxygen and pulmonary rehab.
If you breathe in enough second had smoke, you can develop emphysema.

When you have emphysema, the alveoli lose their elasticity making it difficult to move air.
There is no problem witnessing second hand smoke. The problem comes when you breathe in second hand smoke.
Breathing second hand smoke, however, will make emphysema worse and reduce the amount of oxygen taken in.

any irritant smoke smog dust will cause you to have a more lightheaded feeling due to decreased oxygen because the irritant takes up free space and your alveoli not longer have the ability to stretch
